PERSON CENTERED PLANNING

Person Centered Planning is a service offered to adults served by ASC. It is a process assisting people to identify goals they would like to pursue. A trained facilitator aids the process by assisting individuals to identify a team who will support them in reaching these goals. The team may consist of family members, friends, co-workers and paid support staff. The facilitator ensures the person's dreams, interests and desires are recognized and this helps the individual to make decisions about their life.

Well being is promoted by honouring the whole person's Spiritual, Physical, Intellectual, Cultural, and Emotional self.During the development of the plan, the facilitator ensures the individual's Spiritual, Physical, Intellectual, Cultural and Emotional well-being is respected (SPICE). The planning process results in a written record of the individual's goals, outcomes, actions and timelines as agreed upon by the team. The facilitator follows the individual's progress toward their goals throughout the year, offering feedback to the team.
